Terms and Conditions
-The undersigning Company listed on this application hereby applies for credit in accordance with the terms and conditions of Gulf Coast Safety Council ("GCSC") 170 James Drive East, St. Rose, Louisiana, 70087. -Information submitted will not be shared with or sold to any other entity and will be held in the strictest confidence. -Terms for Payment GCSC is a non-profit corporation, supported entirely through modest annual membership dues and fees assessed for class attendance. All classes are offered at the lowest affordable fees. -If an invoice has not been paid sixty-(60) days following attendance, the respective company will be unable to receive further training from GCSC. Clients can help maintain the low fee structure by processing invoices for prompt payment. -Terms of Payment Acceptance The undersigning Company certifies that all information on this application is correct, terms are understood, and in agreement to proper payment in consideration of extended credit. -Schedule/Cancellation Policy Instructor-Led Courses (Basic Orientation Plus, CPR, etc.) must be canceled by 3:00 PM, one business day prior to class. CBT Courses (General, Site-Specific, etc.) must be canceled by 3:00 PM, the day of the schedule course. Companies will be charged a full price No-Show fee for all Instructor-Led/CBT Courses not attended or canceled. Trainees registered under a false pretense will not be given a refund. Trainees registered with an expired ID will not be given a refund. A Social Security Verification Fee will be charged if applicable. There is no deadline for online registrations, however you must allow ample time for trainees to check-in prior to class. GCSC reserves the right to cancel a course at any time, including but not limited to: lack of participation, classroom, equipment or trainer availability. -ID Requirements All Trainees: 1. Valid U.S. Driver’s License; Valid U.S. State or Federal-Issued Non-Driver Identification Card with Photo; or Valid U.S. Passport First-Time Trainees at GCSC Facilities: 1. Valid U.S. Driver’s License; Valid U.S. State or Federal-Issued Non-Driver Identification Card with Photo; or Valid U.S. Passport AND 2. Original Social Security Card; U.S. State or Federal-Issued Photo Identification Containing Social Security Number; or *Faxed or Emailed Letter on Company Letterhead Gulf Coast Safety Council reserves the right to cancel any registration form for any reason deemed necessary without notice. By choosing I ACCEPT, you agree to the above mentioned conditions. -Non-English Orientation Registration: IMPORTANT: Before registering any employee for Non-English Orientation, you MUST call the registration desk to confirm that classes are being held on the specified day. Please note that some plants do not allow Non-English cards, therefore you are responsible to view the site specific business rules for acceptance rules. Only plant representatives can make exceptions to these requirements in writing. Gulf Coast Safety Council reserves the right to cancel any registration form for any reason deemed necessary without notice.
